Eosinophilic Meningoencephalitis Caused by Paragonimus Westermani Mimicking Brain Tumor

Abstract
Eosinophilic meningitis or meningoencephalitis(EME) is a syndrome characterized by the presence of eosinophils in the cerebrospinal fluid(CSF) of mammals. It is known to be associated with infections of Angiostrongylus cantonensis. The author reports a rare case of EME mimicking brain tumor caused by Paragonimus westermani. A 34-year-old female presenting with severe headache and visual disturbance which was suddenly developed and continued for several minutes was admitted. Peripheral eosinophilia with eosinophilic pleocytosis in the cerebrospinal fluid(CSF) and a single cerebral neoplastic lesion detected by magnetic resonance imaging were observed. After immunologic tests for serum and CSF, a surgical resection for definite diagnosis was performed. The pathological result was eosinophilic meningoencephalitis. Results of enzyme-linked immunosorbent assay for Paragonimus westermani were positive in serum and CSF. And, nodules in left apical and lingular areas were detected by chest computed tomography. After immediate praziquantel and steroid therapy, the patient has attended regular follow-ups without any symptoms.
